The questionnaire is presented on the BPMentalHealth website, and can be completed on a "guest"
computer inside the clinic, or on the patient's home machine. Notice that there are no mechanisms
for identifying the patient on the website. The questionnare ID is correlated with the patient database
in the clinic, after the results have been downloaded to the clinic.

Back in the clinic, an Evaluation Report, generated from the system using
rules and language provided with the PHQ-9.

Although the language on this sample is lifted word-for-word from the Pfizer documentation,
all evaluation rules and language are completely under the clinic's control.

It is technically feasible to create a questionnaire for just about any purpose. If we know the
evaluation rules, we will load up a "standard" version. Rules and language are completely
configurable by the clinic.

We are not allowed to evaluate the PAI for you. It's proprietary. We do, however, 
make provision for you to Export the response to the PAR scoring program:
